Breakthrough for Families works to prevent homelessness by
helping to keep families together through conflicts or crisis by providing
free, intensive family preservation services to high-risk youth (runaways,
street kids, foster kids, teen parents, etc.). Based on the "wraparound" model, Breakthrough for Families provides support that is:
- collaborative
- community-based
- culturally
competent
- individualized
- outcome-based
- strengths-based
- team-based
- unconditional
Our Family Coordinators help youth and families set up their
own wraparound teams that help parents and children commit to making positive
changes and staying together as families:
- Teams include family members, friends, counselors, teachers,
pastors and other professionals or supporters
- Most teams meet for 8-10 months
- Teams use the Individualized Tailored Care or “wraparound”
model of support, providing medical, mental-health, educational, employment
and/or housing guidance and resources
- Teams offer “breakthrough” insights and practical advice helping
families discover and recover their inherent strengths
Most youth and families are referred from other Volunteers
of America programs or other social-service and mental-health programs in the
community. Each year, hundreds of friends, family members and professionals
from dozens of agencies contribute to family support teams.
